FRONTLINE WORKER

The Phoenix Transition Society provides residential programs for women and their children who are at risk of violence or have experienced violence.  In collaboration with Phoenix Transition Society staff, the Society’s Child Support Worker provides a safe, supportive environment that is culturally open and accessible in order to support the personal development of children. The Child Support Worker provides service directly to children by developing and delivering interventions to best support them.

CHILD SUPPORT WORKER

The Child Support Worker provides service directly to children by developing and delivering interventions to best support them.

The Case Management Clinical Nurse provides case management and advocacy for the residents of Harmony House. This role works closely with the Harmony House team, physicians, support workers, and community partners, and provides guidance to residents and staff through case management. This role also offers one to one support for intake and discharge planning for residents accessing or exiting the Harmony House program.

The Case Management Team Leader acts in the capacity as a manager within Harmony House, providing leadership to the front-line team and is responsible for the general management of the team’s activities, procedures, and duties. This role works closely with the Harmony House team, physicians, support workers, and community partners, and provides guidance to residents and staff through case management. This role provides advocacy, one to one support for intake and discharge planning for residents accessing or exiting the Harmony House program. The Team Leader also contributes to the Harmony House management team including undertaking on-call duty to ensure 24 hours management coverage.

The Social Worker provides guidance to residents and staff through case management. Case management includes women-centered support by organizing collaborative meetings, completing referrals, providing advocacy and encouragement, and attaining documentation and certificates of completion for programs and services. The Social Worker liaisons with the MCFD and lawyers for safety planning, family case planning conferences, mediation, family group conferences, court orders, and applications. The position also offers one to one support for intake and discharge planning for residents accessing or exiting the Harmony House program.

The Finance Manager participates in the development of annual budgets, manages and monitors the budgets and financial management systems, tracks legal and contractual responsibilities, maintains financial integrity, and provides accurate and timely financial information and advice to the Directors of Harmony House and Phoenix Transition Society, financial institutions, and Revenue Canada. The Finance Manager performs managerial functions in the Director of Operations absence.
